Our doctors may not always be available at your requested time slots. We sincerely feel sorry for your inconveniency and ask for your kind understanding.

When you call us for your appointment, please tell us the following informations.

Site of injury
Symptoms and when it started.
Record of therapy and treatment in the past
Availability of Referral form, XP, MRI, CT data
Particular requests and/or purposes (surgical operation, rehabilitation, training guidance, second opinion, etc.)

First visit

Though our consultation is based on appointment only policy, we don’t hesitate to spend more time than we initially expect in some cases to exam its specific nature. So please kindly be patient to wait for your turn to come. Normally it takes a few hours to go through an adequate consultation. A pre-exam sheet will help make your process even smoother.
Medical Questionnaire Form
After interviewing, we will do x-ray, MRI, CT scan and others if necessary. We may do these processes even though the data in the past are available along with the referral form.
Please bring your no-sleeve shirts, shorts or whatever so that the site of injury is easily exposed and examined.
Don’t be shy to ask for support from our Medical Fitness Division. They are always willing to help make your recovery even faster.
